In consideration of being allowed to participate in any way in the Armenian Youth Federation (“AYF”) program, related events and activities, the undersigned acknowledges, appreciates, and agrees that:

The risk of injury from the activities or events involved in this program is significant, including the potential for permanent paralysis and death, and while particular rules, equipment, and personal discipline may reduce this risk, the risk of serious injury does exist; and,

Any incident involving illicit drugs will result in the AYFer being sent home at their own expense. Smoking of tobacco will be allowed in designated areas only. There are no second chances; and,

You must always be respectful of your peers, fellow AYFers, and the hosts of the event. The event coordinators have set curfews, event and meal times, as well as basic rules of conduct. You are expected to adhere to these rules. Cursing or talking back to fellow Ungers or Ungerouhis will not be tolerated. Failure to adhere to these simple rules of conduct will result in a disciplinary action. If the behavior persists, the AYFer being sent home at their own expense. Appropriate attire is expected and mandatory at every event. If at any time an AYFer’s attire is deemed inappropriate it must be changed immediately; and,

Everyone at the event is a guest representing the AYF. Should a member be caught in an act of vandalism of property, they will be sent home immediately at their own expense. They may also be subjected to further disciplinary action by the AYF. Furthermore, they will be held financially responsible for all damage resulting from their conduct; and,

Any member removed from the facilities will not receive a refund. All decisions are at the discretion of the organizing body and/or AYF Central Executive; and,

The risks associated with exposure to and infection by COVID-19 or any other communicable disease is significant, and participation in the activities or events will increase the risk that I may be exposed to, contract, or spread COVID-19 or some other communicable disease. I acknowledge that COVID-19 is extremely contagious and can cause serious and potentially life-threatening illness and even death; and,

I acknowledge and knowingly and freely assume all such risks of injury, infection, or loss associated with the activities or events, both known and unknown, even if arising from the negligence, recklessness, or intentional conduct of the releasees or others, and assume full responsibility for my participation; and,

Inappropriate physical contact among members is unacceptable. Those who are caught in violation of this rule will be sent home immediately at their own expense. Any behavior that is deemed bullying will be punished accordingly, and may result in a phone call to a parent/guardian, or dismissal if the behavior is consistent.; and,

I willingly agree to comply with the stated and customary terms and conditions for participation. If, however, I observe any unusual significant hazard during my presence or participation, I will remove myself from participation and bring such to the attention of the nearest official immediately; and,

I, for myself and on behalf of my heirs, assigns, personal representatives and next of kin, hereby release and hold harmless the Armenian Youth Federation (“AYF”), their officers, officials, agents and/or employees, other participants, sponsoring agencies, sponsors, advertisers, and if applicable, owners and lessors of premises used to conduct the event (“Releasees”), with respect to all and any injury, disability, death, or loss or damage to person or property, whether arising from the negligence of the Releasees or otherwise, to the fullest extent permitted by law.

I expressly agree that this waiver is intended to be as broad and inclusive as is permitted by applicable laws, and that if any portion of this waiver is found to be void, invalid, or unenforceable, the remaining portions shall remain in full force and effect.

I HAVE READ THIS RELEASE OF LIABILITY AND ASSUMPTION OF RISK AGREEMENT, fully understand its terms, understand that I have given up substantial rights by signing it, and sign it freely and voluntarily without any inducement.

For Parents/Guardians of participants of minority age (under 18 at time of registration) – if applicable

This is to certify that I, as parent/guardian with legal responsibility for this participant, do consent and agree to his/her release, as provided above, of all the Releasees, and, for myself, heirs, assigns, and next of kin, I release and agree to indemnify and hold harmless the Releasees from any and all liabilities incident to my minor child’s involvement or participation in these programs as provided above, even if arising from their negligence, to the fullest extent permitted by law.
